Required Patron Information

To receive food assistance, households must self-attest that they meet TEFAP income eligibility requirements, which have increased to 200% of the Federal Poverty Level. No income verification documents are required.
Applicants are no longer required to provide proof of residency, such as a driver’s license or utility bill. Instead, they will self-attest by providing their county or zip code. For each household member, we will need full names and birthdates. If there are children under 18 in the home, providing proof of Medicaid, free and/or reduced lunch, PeachCare, WIC, etc., may help qualify for additional supplements if available.

Please follow the link below, scroll down and select English TEFAP Household Eligibility Form or Spanish TEFAP Household Eligibility Form to view the current income requirements.

Food is distributed by the household, and not to each person in the home.
